Chromebook Policy and Insurance Form

In an effort to make Chromebook checkout more convenient, we have made it available online for parents to fill out prior to pickup. BE ADVISED: Students cannot pickup Chromebooks unless the form is filled out by the parent/guardian prior to picking up the Chromebook. In the form, you have the ability to choose if you want insurance for your device or if you choose to opt out. If you choose to pay for insurance, it is $25 payable by cash or check. Checks must be made out to McLoud Public Schools. INSURANCE DOES NOT GO INTO EFFECT UNTIL IT HAS BEEN PAID IN FULL. The cutoff to pay for the insurance is August 10th.
